How To Obtain/Purchase OS X 10.8 Mountain Lion From The Mac App Store

Prepare for OS X 10.8 Mountain Lion.
To be safe:

Restart your computer.
Quit all running applications.
Disconnect all USB/FireWire devices.
Eject all optical discs.
Unmount all servers.

Make sure you are connected to the internet by using a web browser to go to Apple.
Go to Mountain Lion on the Mac App Store .
This should open the Mac App Store application (if it doesn't click the "View in Mac App Store" button).
In the Mac App Store application click the "Buy" button.
Enter your Apple ID and password.
The Mountain Lion installer should start to download (an icon will appear in your Dock with a progress bar).

NOTE

When the Mountain Lion installer has downloaded it will automatically launch and ask you to continue.
Do not click Continue yet.
Quit the Lion installer (yes, quit it).
Go to Macintosh HD > Applications and make a copy of/backup the "Install OS X Mountain Lion" item (4.3GB+ in size) e.g. copy it to another disk or a DVD-R
You may also want to obtain the OS X install disc image.
Go to Macintosh HD > Applications and open "Install OS X Mountain Lion".
Follow the on screen instructions.
Once Mountain Lion has installed you should:

Install the latest OS X 10.8 Combo Update.
Go to Apple menu > Software Update and update all other Apple software (An Apple ID may be required for some updates).
Read the OS X 10.8 Post Install FAQ.
Check the OS X 10.8.x Major Issues List.
Update any Microsoft software you may have installed.
Update any Adobe software you may have installed.
